Treatment Disclaimer:

Scope of Services:

• The clinic agrees to provide the patient with cosmetological services, including but not limited to skin evaluations, diagnoses, treatments, and other related services.

Patient Responsibilities:

• a. The patient agrees to provide accurate and complete information about their medical history, including past and current medical conditions, medications, and allergies.
• b. The patient agrees to follow the physician's recommendations and treatment plans.
• c. The patient understands that any missed appointments or non-compliance with treatment recommendations may affect the outcome of the treatment.

Consent for Treatment:

• The patient hereby consents to treatment, which may include medical procedures, medications, and therapies deemed necessary by the physician for the management of their skin condition.

Fees and Payments:

• The patient agrees to pay all applicable fees for services rendered by the clinic. The clinic will provide the patient with a fee schedule, and payment is due at the time of service, or as otherwise agreed upon.

Privacy and Confidentiality:

• Both the clinic and the patient agree to maintain the confidentiality of the patient's medical information in accordance with applicable laws and regulations.

Cancellation and Rescheduling:

• The patient agrees to provide at least 48 hours' notice if they need to cancel or reschedule an appointment.

Termination:

• a. The clinic reserves the right to terminate the patient's treatment if the patient fails to comply with the terms of this agreement.
• b. The physician reserves the right to terminate the patient’s treatment process if the patient fails to follow physician’s instructions or if the physician decides that the patient’s behavior does not satisfy.

Changes to Terms and Conditions:

• CSeven Skin Clinic (PVT) LTD reserves the right to modify or update these terms and conditions at any time without prior notice. It is your responsibility to review these terms periodically for changes.

Governing Law:

• These terms and conditions are governed by and construed in accordance with the laws of Sri Lanka. Any disputes relating to these terms and conditions will be subject to the exclusive jurisdiction of the courts of Sri Lanka.
